I need an advice from PHACO ophthalmology surgeon

Okay , let me explain what happened.
I am ophthalmologist, and I performed PHACO surgery without any complications during the surgery.
The first three days after the surgery there weren't any problems with the eye, but I saw a VERY SMALL peace of lens material in the anterior chamber.
The fourth day there was cornea edema in the lower part and higher ocular pressure ( 28mmHg ).
On the 8th day - the ocular pressure has normalized ( with therapy that I gave ) , but the cornea edema is worse and it has expended in the central area.
My question is > Does the very small part of the lens that is in the anterior chamber is the reason behind these complications?
Should I remove this LITTLE peace of lens?
I am afraid that I might cause additional stress to the cornea.

Is this a piece of cortex or nucleus, can you tell? If it's a very small cortical piece you can probably still watch a little bit longer with aggressive IOP management and frequent topical steroids. If it's a nuclear piece, it will require another trip to OR for sure. It only happen to me a few times out of my probably 10,000 phacos by now, but my rule if it hasn't gone away in a week or two, I take them back.

plus, it takes all of 30 seconds to get the piece out vs staring at it and going crazy trying to measure its size for the next few weeks...

Thank you all for your help! I did remove the piece, continue with antiglaucomatose therapy, as well as cortico. Now wait to see improvement in the cornea.
