if a Residency on probation, how can we find the reason why?

1. If a residency is on probation, how can we find out the reason why?
2. How can we find out how long the program has been on probation?
 
I looked through the list of programs on probation on the ACGME website and there are no ophtho programs on probation. You can find out probation history though to see if they were on probation in the past.

In my random clicks and searches, Indiana was the one program that had a recent (3-4 years ago) probation.

Apparently there are three categories:

1. Full accreditation/continued full accreditation--these programs are OK
2. Probationary accreditation--seems like these programs have a few problems that the ACGME would like to see rectified before they give it a full approval.
3. Probation--Not good.
