Unless he does something that substantially improves his application, yes, his chances do go down. This is true for all applicants, though, not just IMGs.
Do everything you can to match the first time, because it just goes downhill from there regardless of where you graduated from. The lower down the totem pole you were to begin with, the more steep the fall.

Overall, its not. In this specific situation, it perhaps added a few interviews and he ended up matching. Everyone is expected to pass Step 3. If you fail it, that could be another big red flag in your application.
