If someone doesn't match from the caribbean are there alternate career pathways?

Not a lot. From what I’ve seen, most decent non-clinical gigs (pharma, expert witness, insurance, etc.) generally require at least a year of residency; often times they even want board certified/eligible docs. Maybe you could teach college A&P or something like that? There aren’t going to be any options that are good as far as pay-to-debt goes.
 
This is why we always tell people not to go the Caribbean route. You’re better off not borrowing all that money to be jobless.

I’d also throw in the brand-new scam MD/DO schools into this category as well.

Brand new MD schools are not scams. Brand new DO school like Kansas COM is a scam. LCME is better at accreditation than COCA.
