I'm Asking for an LOR Tomorrow...Somewhat Unclear

Before everyone tells me "use the search feature", I did just that and I'm still unclear.

I want to be as careful as I can with this process, as LORs are very important in an application (as what I've been told).

Anyways, I'm going to ask a dentist tomorrow to write an LOR, and I'd like to know how I should handle it, concerning where I should send it when I get one?

Someone please give me step-by-step instructions from getting an LOR to sending it. Where should it be sent? Who should get it? What are mistakes I should avoid?

If you've shadowed this dentist, then you've probably already told him/her that you're interested in applying to dental school. Ask them for a recommendation letter detailing what you did, what you saw, and how many hours in total you were at their office. You should probably write these points down for the dentist so they don't forget to include anything. Have the dentist place the letter in a sealed envelope (with a signature over the lip).

If you're applying for 2008 entry, then you'll have to print off a matching letter from AADSAS to include with the letter.

If you're applying for 2009 entry, then hang on to it until the applications for 2009 open up. At that time, you'll have to print the matching form and send it off with the letter to AADSAS. The address for AADSAS is on the matching form.
