You would think that those programs that are unfilled would add some more positions on their lists. I mean, to put less than half of filled programs? I'm no rocket scientist, but that seems strange.
 
thats maybe why they went unfilled, They rank fewer applicants per position and better programs match does applicants that they were counting on.
 
It is amazing to me the number of people who don't rank most (if not all) the programs they interview at. Are these programs that bad that they would rather scramble than go there? Frequently the answer is no, they just got cocky. This happens with programs as well. I saw a number of well regarded programs on the unfilled list last year. Did they end up better off by having to accept someone in the scramble or remain unfilled (although the competitive fields generally don't remain unfilled)? Probably not. If they had ranked one or two more people that interviewed there, they could have avoided the hassle. Bottom line rank what you would be willing to take, keeping in mind the alternative may be no match for you.
 
Another factor may be that the umatched applicants didn't have a longer ROL because they didn't get enough interviews for the specialty they were applying for.

You can't rank seven programs if you only interview at five.
 
I am told that some programs dont rank as many on purpose because they would rather get someone from the scramble than get the bottom of their interview list...(How about that for garbage? Waste your time and money for an interview but don't want to rank you.)

Also you have to take into account designated prelim programs. Where they want you to match for a PGY 2 before they hand you a position. Those will always be deceptively unfilled by match time.
