Is it okay/appropriate to have a specific personal statement for a specific program that says something like "I want to live in XYZ City (ex. because my significant other is a resident physician in this city) and have a special interest in XYZ program"?

Also, where on my app should I put a submitted case report. I read in a previous thread that submitted things should go under the "peer reviewed journal article/abstract (other than published)" category, but what if it's a case report?

You can, but in most specialties it is not expected. (Notable exception is ENT.) Personally, I'd much rather read about why you're interested in the specialty.
