If something needs to be disclosed on your application, disclose it. It can majorly bite you in the butt if you don't.
 
Just be honest with them. Just don't make excuses for anything and tell them what you learned from the experience.
 
Agreed. If you have something you need to disclose on the application, just do it. If you don't disclose everything and it is somehow uncovered in the future (even after you finish school) you can have your license revoked.

For what it's worth, I made a stupid mistake sophomore year...I disclosed it and was still invited to interview at each school I applied to. They didn't even ask about it. Schools know mistakes happen.
