Looking for some advice here, hopefully you guys can help me out.

So for the past two years I have been pretty off track. I was originally doing well in school, but I had a very close family member pass away and that really knocked me off emotionally. I decided, stupidly, to continue even why my academic advisor and family told me to take some time off. I went through the last two years taking a bunch of W’s because it was very hard to focus on schooling. At the same time my dad lost his job and my mom became disabled, this forced me to work around 40 -45 hours a week under the table, but recently I secured a full-time job, a nurses assistant at a top hospital. I’m not sure if I should take a year off and work there full time and then return to school, or decline the offer and go to school full-time and try to get back on track on what I was doing previously. I could also take a couple classes part-time while working. I’m not really sure.

If I was in your situation, I would probably take a year off and do some work. If your head isn't in school and you aren't able to focus right now/for the past two years, maybe a break is what you need. I suggest talking to your academic advisor again. It comes down to what you think would make you successful...if you think you can get back on track, maybe go that route...but if you haven't been able to do so for the past two years, maybe a break would be better. I would do a semester off and work full time (no classes), and see how it goes. If you miss having school, and a part time class the next quarter/semester. Or, brush up on the courses you took W's in, since you'll have to take them again eventually

Just my two cents. You know how you are feeling better than others. Do whatever is best for your overall well-being. Getting into med school and becoming a successful physician is a marathon, not a sprint. A year isn't that long in the grand scheme of things, especially when it will put you in a better mental state.

A year off to get things together would seem very valuable rather than trying to finish school amid chaos.

I suggest working, saving money, and then go back to school when you're 100% ready.

The work experience alone is a good to have.
