As part of printing out the matching forms, you must place your instructor's address within the required fields. What do I do if I don't know the instructor's address?

You could put your instructor's office address at your school (or the address of the building they work in). Or, you could also send a quick e-mail to ask your instructor for his/her address. Hope this helps!

Sorry this doesn't answer your question, but do we have to use interfolio? What's the benefit? I was planning on just inputing my evaluator's email and name into AADSAS and I thought they'll email my evaluator with steps to submit the letter.

Nope, you don't have to use Interfolio--it's completely optional! It's just a great way to store your letters for the future. You can definitely go ahead and just ask your evaluator to submit directly to AADSAS.
