interfolio questions again

I know that this question has been asked many times but I am STILL confused...
sorry for asking the same question again.

So, some of my reference letters are done, according to my professor and dentist.
And they are asking me what they should next.
I purchased an interfolio account. so
1) click on request a letter
2) select AADSAS letter of Evaluation form
3) format should be paper document (according to AADSAS)

but what does this mean under AADSAS website:
"We encourage you to include the the Letter of Evaluation Matching Form with each letter for faster processing. "
where can I find this letter of evaluation matching form?

please help what I should do next!

The matching form will become available once the next cycle opens up; I think it will open up around June 3rd. The form will be inside your AADSAS portal when you set up the account once the cycle opens up.
 
so for now,
I just send the letter request to my referees, but that's it though right?
All they gotta do is submit the LOR and they don't have to do anything afterwards?
matching form is done by me right? I don't have to contact them later?

You can request the letter now. Once the portal opens and you get the matching form, you will need to get their signature on the matching form if I recall correctly.

should I ask the professors/dentist to NOT include the date so that I can reuse it later if necessary? Or is the date required to put in the LOR itself?
Also, they need to add their signature into LOR correct?
And also when I am requesting the LOR, I have to select confidential NOT AADSAS option
right?

Are you sure the matching form requires THEIR signature?

To my knowledge, you print out your 4 LOE matching forms., fill out the forms, upload to Interfolio, and then send out as a 'delivery' your 4 LORs with your 4 LOE matching forms that you just uploaded into Interfolio.

I was not aware that professors had to sign the matching forms. This sort of defeats the purpose of Interfolio, because you'd still have to track them down after the app cycle opens to get them to sign. Can anyone confirm that this is the case? I'm thinking it isn't.
