does anyone know what are the best recommendation for preparing for the internal med. shelf?

If you do a search for "internal med shelf" you will probably find a couple of threads that discuss the books used for internal med in detail. I think the general consensus is to use Step-Up throughout the rotation and get through as much of it as you can (no small feat) and then use MKSAP or Kaplan QBook/QBank/USMLEWorld for questions (the former if you just want to buy a book, the latter if you're into spending larger amounts of money for a question bank).
 
Medicine was the worst shelf for me for giving the questions where you get 5 answers, 3 of them are "correct", but you have to choose the "most" correct. Often times this is based on doing something first in an emergency situation or doing an initial screen out test that's cheaper than other options. So focus on stuff like this during your reading and clinical rotations.
