how much can really ambitious/busy internal medicine docs make? yeah i know the average is low esp. compared to other specialties...but the IMs who i know in private practice also dont work as many hours....
can you make over 200?

a resource you should check out is merritt hawkins & associates (www.merritthawkins.com). they just released 2007 data (about 2006 compensation): http://www.merritthawkins.com/pdf/2007_survey_primarycare.pdf

a rule of thumb for salary, in primary care at least, is that you need to produce roughly 2-2.5 times of what you expect to make. so, if you expect to make 150k, you need to produce a revenue stream of 300-375k. if you expect to make 200k, you need to produce a revenue stream of roughly 400-500k.

in cali, the first year out of residency (im, given that we're in the im forum) offers for outpatient practice are in the neighborhood of 135k. on the east coast, the offer may be a little higher, around 145k. most practices are going to expect a newly minted attending to see 12 patients per half day (24 patients/day).

200k is possible, but there's likely to be a workload comensurate with that... or joining a group practice that understands cost containment, and keeps the overhead low.
 
Kaiser on the west coast offers about 150K to new grads in IM..
 
In order to make that much and keep workload low, seeking a hospitalist position might be the best bet. There are a few places in the east coast that offer 250K for a 7-on-7-off schedule.
 
if you are very efficient and are able to digest a large volume of patients you can make 150-200k/year

if on top of that, instead of referring out ultrasounds and other imaging, you just set-up an imaging suite in your office and arrange for a radiologist to do the readings - then you can probably add another 300-600k/year (of course you need to see 40 patients a day to generate the volume necessary for imaging orders - ie: if you only order 5 ultrasounds a month then forget it)... then add stress tests, nuclear imaging, etc... you can make even more

but basically without being an entrepreneur you will rarely see more than 200k/year.
 
I personally know of a very busy, older IM doc who saw a new patient every 30 seconds, got a share of laboratory revenues, and paid his MA's/LPN's minimum wage with no health benefits, and cleared well over 300-350K every year.

It CAN be done, as long as you're willing to put money about family, home, and human decency.
