If I am interested in interventional neurology but I match into a program that doesn't really offer it, what can one do to prepare for applying to this fellowship? I heard that it is one of the more competitive neuro fellowships (along with pain/sleep). Is "regular" stroke fellowship relatively easy to secure then? Thank you.

It is not impossible but it will be an uphill battle. Remember, going to a place with name faculty, especially within the INR field, will probably give you a leg up. INR is a small field and a good letter from an INR would probably go a long way.

You should go somewhere with strong stroke and neurocritical care training if you dont get into your preferred program(s).

Stroke is not too difficult and I believe it is part of the neurointerventional track. So you could match into your preferred program's stroke program if not for residency. You need find stroke faculty who have connections to neurology trained neurointerventionalist for your fellowship.

Dont sweat fellowship too much. You may change your mind and you really haven't done stroke call or neurology training yet. This is like going into med school with a predetermined residency choice.

Unless you know you absolutely have to do this fellowship (and call for life), focus on going to a well rounded residency with well known faculty who can help you get into any fellowship, anywhere.
