Is a doctor's administrative work done during working or after he finishes work ?

This is difficult to answer and will depend on the type of physician and the type of medical practice (academic medicine vs. private practice). In academic medicine you see fewer patients and have protected time for other activities such as teaching medical students and residents or administrative duties like hospital committees. Physicians in private practice are focused on seeing patients and have less time devoted to other things. They are more likely to have to balance any administrative duties with their full clinical schedule.

lets just consider those that work in public hospitals .

This is still difficult to answer. Physicians that work in hospitals are often employed by the hospital or healthcare system but not always. If they have administrative work as a portion of their assigned duties, then they will likely have some protected time during their work day to complete it. How they manage this work is variable and would not likely be generalizable to all physicians.
