Giving my opinion since this thread looked lonely:

I was just an applicant this year so I have no actual idea, but I imagine that you'd have a hard time selling this. The reasons I've seen that medical schools will consider for a deferral are pretty much only international scholarships (Rhodes, etc.), and I think MD-only programs can afford to be more lenient since they have a much higher number of incoming students (>100 c.f. ~10). So I think it's less likely that an MD/PhD program would let you defer, especially for a less compelling reason.

And it's a bad idea. You will be in training until at least your mid-30's at the earliest. Why procrastinate?
