5 years ago, in a programming class, I copied some code from github (a code sharing website) and submitted the assignment. It was flagged by the autograder, and I received a zero on the project.

There was no meeting with any professors, or even TAs... they didn't even contact me. It was just flagged as "parts copied from github," and the grade on the project was just a zero. Didn't affect anything else in the class, and that was the end of it. There is no record of it on my transcript/student conduct, didn't meet with a single person about it... just received a zero on the project.

I understand this was really stupid of me, and I haven't done anything like it since, but does this need to be reported?

If there’s no record, you don’t need to report it. And you shouldn’t.
 
You dodged a bullet there, @IcedCoffeeOnly. Hope you learned your lesson. A repeat of that behavior could put your medical school hopes on ice so don't do that sort of thing again or you'll be in hot water.

Curious, since stuff like this happened like clockwork during my time at Cal... staff would reuse homework problems, people would get a hold of old problems on CourseHero/Chegg (someone spent 40 bucks on a coursehero subscription for ONE PROBLEM in ONE HOMEWORK lol... why not just do it?), they’d get caught, get zeroes, but nothing really came out of it other than that.

One of my roommates junior year got flagged in THREE different classes, all for copying old homework, but only got a zero on them and never met any of the staff members. It seemed like they only cared about exam cheating (which they took extremely seriously) and let homework/“minor” project cheating “slide with a 0.”

I guess it has consequences since the zeroes would lead to a low grade and the students wouldn’t be able to declare CS as a major, but it seemed like people were more like “oh dang I got caught again, oh well” instead of “oh sh** what am I gonna do?”

If these people were to apply to med school, would they still not have to report any of it, since there was no “official action” taken against them? Even if they got flagged 3 times...?

If so, that seems kind of... BS.
 
No, and this is one of the many reasons why life isn't fair

Yeah... funny how someone can make a mistake with a citation and get written up for it, while someone else can buy their way through a class, get caught, and go "woops."

Can only blame my ancestors for not being of royal blood and not buying me a silver spoon at birth.
