So, you should really read the 18-19 applicant sticky. In particular, check the first page of the official AMCAS questions thread to find the AAMC videos on filling out AMCAS. Will answer most of your questions and more. The straight answer is that you should fill in coursework EXACTLY as it appears on your transcript.
