Anecdotally Ive seen Decadron IV given po for the chillens. Doc asked me about Zantac, googled and found some places do IV Zofran po.

Anyone have any experience with this? Id like to get a list together if possible.

The obvious one that comes to mind is vancomycin powder for injection given orally for C diff. (remember it can't cross the GI tract).

We use IV Bumex orally.

edit: vanco as well

Heres the article that confirms they used IV decadron PO.... http://www.nejm.org/doi/full/10.1056/NEJMoa071255#t=article

Thanks for the article! I was looking for information about this the other day.

It would be interesting to find a list or something for IV to PO. I was reading another reference that said the two main things to consider when attempting to convert a product are 1) bioavailability and 2) other preservatives or excipients in the product.

If it can be squirted directly into your bloodstream, I figure it can be patted down by TSA on the way to the intestines. If it's available PO from the manufacturer, I'd imagine it's fine squirting it down the throat at the same PO dosage if the PO dosage is also non enteric coated and not extended release. So zantac and decadron would be fine. As would, say, diphenhydramine.

You just look at the difference of absorption more or less for everything else. Vanc isn't absorbed, so obviously you can't switch to that for systemic issues.

The Narcan and Vanco are exactly what Id want to avoid when giving other drugs po, they dont work systemically when given po which is why they are used that route.

I just remembered NAC we give orally.....asked around and others have heard of Zantac and Zofran. Hmmm....I may try and write something up. Good for our stand alone ED with limited meds who sees a bunch of kids.
