J1 visa without statement of need - is that possible?

Hey guys. Hope you are all fine. Quick pre-history: I'm originally from a country with some "specific political direction" due to which it became physically impossible for me to obtain statement of need from ministry of health-care. ECFMG is requested statement of need in order to start J1 visa process. From here, I have a quick question. Is that possible to get J1 without having statement of need ?
Looking forward for yout advice, what should I do ?

You're going to need to contact the ECFMG about this. If your home country has no functioning government (Somalia? etc), they should be able to tell you what you need to do. If your home country does have a functioning health ministry and you can't get one, you probably can't get a J visa.

In any case, contact ECFMG and ask.
