I was in a meeting with our pharmacy today, and was told that the Joint Commision is planning on requiring a treatment protocol for every off-label use of every medication given in the hospital.

Pause for a minute to consider the ramifications...

I can't wait to have nurses telling me I can't give cardizem for A. Fib w/RVR (only indicated for chronic stable angina or angina secondary to coronary spasm) or reglan for migraines. I'm so pissed about this I can't even think anymore about it.

Looks like it may be time to initiate my backup plan. Australia/New Zealand is looking better and better......

For someone that I gather is not too fond of Government involvement in healthcare is this really a good back-up? I worked in NZ and it is very government regulated.

Speaking of which, what was it like? I've heard great things about the country and, after hearing you were headed down there, have been thinking about it for when my kids are a bit older.

Arrg! How can we get the point across to the JC that forcing us to spend so much time jumping through the hoops of their "safety measures" seriously detracts from the time I am able to spend actually thinking about what I'm doing and thereby makes things more dangerous for the patients?

Sorry to butt in, but was there any memo from the JC about this, or was it kind of through the grapevine? I can't imagine the insanity that would result from a policy like that. Just thinking of the amount of antibiotics that are used for unapproved usages is making my head spin...

Right, but I figure if I have to work in a government system (eventually coming to the U.S.) I might as well work in a place that has tort reform too.

I did a quick google search and didn't turn up anything, but I know our pharmacy's taking it very seriously. Some of the people from the Sermo community also report dealing with this, as well as possible work-arounds.
