I am getting close to getting final offers from a rural hospital and a top psychiatry program. Top psychiatry academic program is probably going to be around $180,000 a year while the rural hospital around $300,000 or slightly more. Rural also has stipend and sign on bonus. My concern is turning away the big name academic institution. The big name and connections may propel my career further in the long run. I do have about $300,000 in loans but know that either way it will be paid off, one way or the other. My other thought is that you can go from top academic to rural but it will be hard to do the opposite later. Thank you for all your advice ahead of time

If you’re picking a job to avoid closing doors, consider whether the opportunities you might lose are ones you care about. If you think you might one day like to be a clinician at an academic medical center, going rural will have no impact on your prospects as there continue to be shortages. If you’re hoping to become a training director, or pursue a funded research career, you’ll be on the back foot. Right now I know of several prestigious academic medical centers that are desperate for clinician educators. Being away from this setting for a few years is certainly not disqualifying.
