keeping track of hours/asking for LOR?

im in the middle of doing some various office shadowing.

there is a gp, endo, and oral surgeon who have agreed to let me observe them for a few days. im hoping to get about 25 hrs out of it.

ive been keeping a little notepad with the date and time/amount of hours spent in the office (and also describiing various procedures etc)
-is this a good way of keeping all my hours tracked? do i need the dr to sign off on it (i dunno if schools want to check for honesty)

and how do i go about getting a LOR. do schools want a sealed LOR. and can the same LOR be used to apply for a number of d-schools? i would hate to ask them to write 10 different letter. copies ok?
-i wont apply till july but i would rather have them give me letters now after i finish shadwoing while they still remember me and not 4 months later when they may not even rememeber who i am lol.

also i have had a wonderful experience with these dentists and i would like to send a little thank you gift as i complete my shadowing. any recommednations? i wanted to order those edible arrangment fruit baskets but theyre like 60 bucks each! too much for a student on budget!😱
 
I think most schools just rely on you to be honest and truthful.

As far as gifts, you could just get some cookies or cupcakes or cake, or something like that.

impressive dat scores btw. how long did you study for? im taking it in jan but ill have to self teach the ochem material (was wailisted)
