Keeping Track of Missed Q's

Hello all, I have read lots of suggestions about studying for the MCAT and on common theme is keeping track of missed questions. I am thinking mainly discrete. MY question is what did you include on your spreadsheet? I actually wrote out the question, named the topic, subject and then make an answer sheet on another spreadsheet. How are you guys doing this? Should I be making them into notecards and categorizing them by topic? How often do you go over missed questions?

I just take a notebook and write out all the concepts around the questions I got wrong. I try to use my own words and try to be as detailed as possible. Some times it's just a bullet point, sometimes a full page diagram. I don't categorize them or anything, but weirdly, it really helped tie together all different concepts for me. And it's a great study material. I could just flip through my notebook and quiz myself on random facts/concepts.
