is applying for a transfer for a PGY-2 position at a program you did not receive an interview at for a PGY-1 position during the match worth the risk of trying to transfer (or strain relations at your home program) to be closer to spouse? the program i am looking at that accepts transfer adds a resident in PGY-2 year.

I would also talk to other programs nearby, maybe if you do a good job in PGY1 and can get the backing of your program director you can transfer to one of those.

Of all the reasons to want to change residency programs, moving closer to one's spouse is a legit reason. Just be positive and do good work, establish yourself as a good resident/colleague in PGY1 and let your attendings get to know you as a solid person and trainee first before bringing it up. Don't wait until the last minute. Usually a good time to talk about it is during annual evaluation, prior to new contracts going out or when you need to apply for the PGY2 spot. Positivity and confidence and solid work as a PGY1 and good timing are going to be key to pull off a transfer, as you'll need your PDs good will.

The few times I have filled a PGY-II opening, my pickiness was greatly reduced. Wolfvgang22 is right, moving to be with spouse is one of the best stories. The field of PGY-II candidates can involve a lot of funky stories.

I think the general consensus is that matching into PGY-2 is less competitive, but if you are aiming for one spot at one particular program, maybe harder depending on the program. Either way, I think moving to be with a spouse is absolutely a great reason for trying to transfer.
