Last name change after graduating from medical school

So I tried to search through the treads on here about what steps need to be undertaken, but was unable to find anything of merit.


Just curious if anyone knows what things need to be done in order to change ones last name to a hyphenated last name after getting married. I have a good idea of the process that is required for the gov't (social security/passport/etc). However; I have not seen anything about what needs to be done after someone has already started residency and received a diploma with their last name on it (Before finishing residency and getting licensed).


~It isn't necessary to discuss changing a name after marriage and the pro's/con's that come along with it....pretty sure that topic has been exhausted on other treads.

Start with the government to get your name legally changed.

Then talk to your hospital, state medical board, DEA, and anything else that has you under any kind of license.

I started medical school with one last name (ex-husband, kids, debt, etc attached to it) (H)
Got married in medical school to husband #2. (V)

My med school diploma only has name #2 (V)
My residency diplome only has name #2 (V)
It's up to you to decide what name you want on your diplomas (I didn't want my ex-husband to have any credit)

I change my SSN to a hyphenated last name due to kids, etc. so H-V
My passport has H-V
My driver's license has H-V
All my state medical licenses are H-V
My DEA is H-V
My DPS is H-V
My NV Pharm is H-V
etc.

Professionally I go by Dr. V to keep it simple for the staff but the credentialling office, etc has to know your full name so it matches your licensing.
