Let's say you were doing an extraction and the root(s) fractured at the midline. They are close to sinus or IAN and there were no periapical pathosis to begin with. Do you go after them everytime or Would you leave them alone and watch it? What would be the prognosis/payoff for either?

At my school the philosophy seems to be to stop just short of maxillary resection when retrieving root tips.

its really a case by case clinical judgement scenario.... I don't believe that in the thousands of teeth that have crossed the path of my cold surgical steel, I have ever left a root-tip intentionally.

I've seen an impacted 3rd molar sectioned in such as a way to leave the roots wrapped around the IAN, but the majority of the tooth taken out....
