I hand-wrote in the space they provided and attached another typed-page.
 
They sent me a booklet and at the end it has the physician eval form--(which can be replaced by a LOR) which we aren't supposed to send--I mean we could be checking those boxes. I dont want to turn my whole app in to the DO and then it ends up not being sent.

Is everyone else downloading that particular from and sending it to the DO, and keeping the booklet? I just don't want anything to get lost!!

Yeah, for the theme of the essay there is VERY little space to write on so I am assuming the norm is to paper clip it to the top if using the booklet secondary?

How long are the essays supposed to be?

I tore my booklet apart and sent only the pages they needed followed with my added sheet (it is perforated). As far as the eval form, I gave it to my DO and told them they could use their existing letter in its place.
 
Just curious if you guys typed your secondaries up or hand-wrote them (Name, social, employment, etc.) I called to see what was preferred and all I got was " it depends on how you want your application presented". Thanks.

Since no one knew for sure I just called and they said that the majority of what they have received have been hand-written. Also, they said it is absolutely OK to attach a seperate typed page for the essays and just to indicate "see attached" on the space provided.
 
I have already submitted my secondary application, and wrote the name of the DO that I THOUGHT would fill out the evaluation, however it turns out that she ended up in the ER and she is sick and wont be able to fill it out for a long time. Do you guys think it would be bad to call LECOM and let them know that this happened and that I might be having someone else fill out the evaluation? Do you think it will look bad on my app? I am soo confused, please help! Thank You!

Okay, now I see, the DO forms are perforated and I tore them out. I typed my essay and will paper clip it or something. My essay is almost 2 pages. Hope it is not too long.

I dont think it will look bad, either wait until she comes back, or call and tell them that someone different will be filling it out. You can help of spontaneous events and your evaluator getting sick.
