Why the discrepancy between intern and categorical? Don't interns just do the same first year as categoricals???

Essentially yes. But it has more to do with the fact that the prelims will be there for one year and then go on to their sweet Derm, Rads, Rad Onc, Ophtho gigs and be done with it. The categoricals will be stuck with the place for 3 years and the decreased fellowship options it offers.
