A professor that wrote me a letter of rec did not put it on official letterhead. One of the schools that i am applying to requires official letterhead. Does the professor just update the letter? or I have to create another letter request form?

You can call amcas, then they will let your prof resend his letter and replace the old one. If you create a new LOR request, that will work also. But, you will get a bunch of annoying e-mails from amcas reminding you to designate the old letter. Which obviously you do not want to do, because the old letter is messed up.

Just call amcas tomorrow and have your writer send a new one.
