Is there a rule out there for Level II trauma hospitals that states any trauma service patient in the ICU that is postop MUST go straight to the ICU, regardless of intubation status? supposedly because the accreditation body requires it?

If it is, it sure seems like a stupid thing to do.
 
That rule doesn’t exist for Level 1 institutions (traumas can be downgraded in real time at most institutions), why would it exist at level 2s? Also, typically the community Level 2 center will punt anything and everything that looks/smells serious to the Level 1 (before they even arrive at the EMS level) especially if it’s the middle of the night when specialists aren’t typically in house

I am sure it is institution-dependent. Most centers will do a 24 hour admit for monitoring with traumas but this can be done on the floor with telemetry if the patient is stable and FAST negative/pan scan negative.

That’s my experience with 2 Level 1 centers.
 
seconding the above... no one should decide icu status of a patient except the surgeon or anesthesiologist
 
We have trauma service patients from SICU go to the floor postop all the time. No such rule afaik.

We have a level 2 trauma service. I am pretty familiar with the protocols..

To answer your question: No, we routinely have trauma post op patients go to the PACU before the ICU if they are non-intubated. Including anything, GSW, craniotomy, MVA, to the PACu and then SICU if non intubated..

I believe the protocol for the surgeons here is that any trauma patient requiring surgery will be admitted from PACU to at least the Step Down Unit or above..
