Listing Activities in Application?

I have a somewhat unique situation and I was wondering how I would list this on Med School apps.

I will be volunteering with different programs that all have to do with the homeless and homelessness. Would I be able to list this all under one category in the app? Also, I heard that there was a place for description so how exactly would do that?
