Listing scholarship on application

I've received the same scholarship for three straight years..

For month and date, should I put the most recent one? This would make the most sense, and am just making sure.

I had the same problem...I just listed the most recent one even though that doesn't tell them that you got it for multiple years. AADSAS should have more options for this section....but I guess this isn't going to make or break your application.
