Listing Shadowing

How should I go about listing shadowing?
I shadowed 4 different doctors so I am not sure if I should put them all into the same Experience or 4 different experiences.
They were all at different locations so all of the contact info is different for each one.

I listed them as one experience and in the description I indicated how many hours I spent with each physician/specialty.

What did you put for Organization Name and the address?
For start and end dates, did you just use the very first shadowing experience and the very last shadowing experience?

I didn't put anything down for organization name and address, I just wrote Physician Shadow in the Title section. For start and end dates, I used dates of first and last shadowing experiences.
