So my religious leader has agreed to write the letter of recommendation for Loma Linda's supp application. He asked me what to write in the letter and how it should be written. He specifically asked me how to start and end the letter (specifically "To whom it may concern, or Dear Loma Linda University School of Dentistry Admissions" and Sincerely or Thank you for closing it out).

I am not familiar with this because I am a student and I have waived my rights. Anyone know how it should be formatted. Should there be a letterhead as well? Is there a character limit?

To Whom It May Be Concern is always good to go.
 
Being that the letter is going specifically to LLU, you could certainly have the salutation specifically to "Loma Linda School of Dental Medicine Admissions Committee," but in the end, I don't think it matters whatsoever. Either is fine.
