I shadowed a long time friend who is a anesthesiologist and asked for a LOE. Does the long friendship before the shadowing act like a conflict of interest? And can he mention character growth over time, or would it be best to stick to the period of shadowing?

Physician letter generally don't hold much weight either way (unless the physician was a professor/PI/employer of yours), but if you're applying to one of the few schools that require one, consider it along the lines of using a personal reference vs professional reference for a job application. Professional reference>personal.
