Looking for a PDF chart with all the optometry schools in the US..

The chart listed all the optometry schools in the US along with average OAT scores, the number of applicants, the number accepted, tuition, whether a bachelors degree was required, etc. I came across it a little while back, but now I can't seem to find it.

Does anyone have it? Or does anyone know where I could get it?
