Looking for free quiz generating software

I'm making some practice quizzes, and I'd like to post them online. I'd love the following features:

• feedback for incorrect/correct responses
• MC, T/F, and fill-in-the-blank support
• image support (for histo)
• ability to choose questions from a bank (say you only wanted to study head/neck) or mix and match (study all anatomy or all extremities)

Does anyone know of any program/site that can do some of this?

I've tried Google Docs (I can't edit forms after I finish them the first time, and people can't get feedback), Hot Potatoes (turns out it's not free), and Wondershare (difficult to get the quizzes online).
