At a glance, what's killing your app is a lack of research, non-clinical volunteering, the MCATs. However, what contributed the most to an unsuccessful first cycle was definitely applying to only 9 schools. Applying broadly next cycle will help your chances greatly, but you should still try to get some non-clinical volunteering hours in.
