LoR after semester ended

Hi guys, I'm wondering how should I structure an email to my prof for a Lor as my uni is done and he's not teaching any summer courses. As far as I'm aware, I need to put a short description of my accomplishments and about myself in the email, and the ask if he can write a "strong" Lor.

I have been to his office a few times and he definitely remembers me, and I got great marks in his class if that matters.

I haven't finished my personal statement so I don't want to include that.

Any advice appreciated, thanks
 
It's better to do these things in person. Shoot them an email asking if they're available to meet with you over the summer. If not then you can ask via email in your follow up email.
 
Erring on the side of caution and not sounding presumptious I wouldn’t include anything in the first email asking to meet him/ asking for LOR like your resume, transcript, etc. until you get confirmation that he is willing to write a LOR. Then you can ask if there are any specific documents he would like and send them for his reference
