LOR: confirm or ask?

I am planning to ask 1-2 of my former professors and a professor I currently have for LOR to medical school.

Should I email them/talk to them and ask them for a letter (providing a cover letter, resume, etc) OR should I just ask them if they would be interested in writing one in the future? I apply next year so is it better to show medical schools old letters from 1-2 years earlier or just get confirmation first, and then formally request them to write it when I apply?

I plan to include a packet, which has a resume and cover letter detailing what I learned from their class- non-material stuff, how their class has helped/could help me in my future. Is this fine?
 
Just ask them for a letter. Use a letter service like Interfolio or the one provided by your school (if they have one) to hold onto the letters until you need them to send them to AMCAS. Unless you're close to those professors, you need them to write them while they still remember you and can write a better letter.
