Is it required to have at least one letter from an anesthesiologist? Does anybody know?

Essentially yes.

I interviewed at one program and they called me later to say they liked me, but hadn't received a letter from an anesthesiologist. It was a clerical problem on their end that was easily resolved, but proved while some programs may not say it explicitly, they are looking for such a letter.

A letter from an anesthesiologist also carries more meaning -- it is saying I'm willing to work with this guy/gal in future as a colleague.
