I worked in a laboratory at my school's nationally recognized hospital for research, but we mutually agreed to not continue because he knew that I didn't have much of an interest in bench work. He seemed to have thought very highly of me, however, and offered to help me find further research opportunities around the area that would suit my interests better, though, and he wants me to let him know what I plan to do in research within the next few days.

I won't be applying for medical school for at least two years, but would it be a good idea to approach him for a letter even if he knew I wasn't too passionate about the material itself?

I'd discuss it with him. It can't hurt. Worst case scenario you don't send the letter.

If you intend to include this experience into your primary application, yes. Try.

Some schools actually want contact information during secondary.

Personally, I would just find a different research opportunity and get a letter of rec from that instead. I was in a similar situation!
