LOR matching form and Interfolio?

I'm really confused with this process. How do I see the "Print LOR Matching Form"? Whenever I set it as "paper" it asks for my evaluator's address, do I have to enter their address? And do I mark it as a committee letter?

on interfolio i think we click "add" where you have the list of your letters, then there is an option to add the "AADSAS Letter of Evaluation Form", and then just upload it on there along with the letters
