LOR matching form question

For the LOR matching form can we fill the whole thing out or do we need to get the person writing the letter to fille out the evaluator section? My problem is that two people writing letters for me are in other counties and I cannot physically give them the form. I could go through mail but that is a pain.

You can fill the matching form yourself if you have all the info.

Be aware that the form has to be attached to the letter. So if they are in another country, they will need the form anyways. An easy way to do this is email them the pdf of the form, along with your info that needs to be filled. That way they can mail the letter and the form togeather.
