LOR Matching Forms Signature?

When I asked my Professors for LORs to be sent to my schools career services, I told them that I would need them to sign a matching form when the application opens. However, I don't seen a place for their signatures? Do I need them to do anything with the matching form or can I take it right to my school's career services?
