Hi all,
I was wondering how matching forms for LOR's work? I saw some old posts but wanted to make sure that it hasn't been changed recently? Do we need signatures on the forms because that would mean tracking down everyone yet again 😱
 
Well, the easiest way is to not use the matching forms at all, and you just put in your letter writer's email address and AADSAS sends them a link to a page where they attach the word document of your letter and presto, all done.

But, if you need to do the matching form, it's just like the transcript matching form - you indicate that this will be a paper submission instead of online, and then it lets you print out a special form with a bar code and your information on it. Then you give it to the person writing your letter and they including it in with your letter when they mail it in.

You'd really need to give the forms to the people so they can send them out themselves if you're not doing it online, because you more or less need to waive the right to view the letters for all of them, or else the schools might not look at them as seriously.

Thanks! But, ok, so I'm doing it at Berkeley through the Career Center and I already have waiver forms saying the letters are confidential, etc. etc. I already have my letters so electronic is kind of not an option. My question is, and I looked at old posts and saw conflicting answers, what is actually on the matching form? Can anyone post their old one (w/ all their personal details, etc. blocked out)? I'm mostly concerned that I would need a signature of the letter writer because that would mean tracking everyone down in the summer. I called the Career Center and she said I could just hand-deliver the matching forms to the Career Center and they would include it with my LOR, but she didn't know what's actually on the matching form. Phew . . .thanks!
 
Oh, if your school is doing it, that's a completely different story. My school does something similar (but I didn't use it because I'd heard bad things about it), and it counts as a "Committee Letter" which you can designate separately on AADSAS. All you'll need to do then (I'm pretty sure) is print out the "Committee" LOR matching form and just hand it to the people in the office to send in with your package of letters.
