For LOR's is it sufficient enough to have committee letter and a recommendation from a dentist, would those two suffice? Also for the personal statement is it ok to send the same one you wrote to the health committee of your school or do you have to write a completely new one? Sorry freaking out here...

Having a committee letter and a letter from a dentist is plenty/what is recommended...as far as the personal statement is concerned, as long as your letter clearly stated your desire to pursue dentistry and gave examples/reasons for why schools should accept you then you should be in good shape.
