I know these are probably questions that I should know but my professor wants to know and I'm not sure how to answer them correctly. I want to be an orthodontic, so should I put D.D.S as the program or...?

1.To know the program/school you’re applying to;

2.To know whom I should address the letter

You are applying to dental school. If you want to be specific you can do DDS/DMD but remember that all school see the letters you add to AADSAS so you don't want to put DDS if you're also applying to DMD programs.

Address the letter "to whom it may concern" for the same reason
 
Do not include ANYTHING about being an orthodontist in your LOR. You attend dental school to become a GP. Any further training, aka not the program you are applying to, should not be mentioned.
 
You will be digging yourself into a hole if you said that you want to become an orthodontist to the dental schools. I know that they don't like that. Your letters will be sent to all of the schools so the letters shouldn't be school specific.

So under programs, I should say "General Dentist"?

I would think generic "admissions committee members" "admissions officers" would work. Or "to whom it may concern"
I seriously doubt this would affect the outcome of your application in a big way.
