I want to add a school that requires a letter from a practicing physician. Unfortunately, I will not have this letter sent in until next week. Should I wait to add this school on AMCAS, or add it and just designate the letter choice on AMCAS?

I'd add it now, you generally don't need your letters in to receive a secondary (or even to submit them for that matter)
